Cheat sheet — Organization Theory, Design and Failure

This theme covers the organisation-theory literatures that either rediscovered the VSM’s logic independently, supply rival mechanisms for the outcomes the VSM claims to explain, or document failures the VSM cannot name. v02 §3.3 rebuilt the “neighbouring literatures” from a convenience sample into a survey, and most of the additions are here: survival as selection rather than design, accidents as consequences of structure rather than pathologies, silence at the sender rather than failure in the channel, competency traps that coordination accelerates, and Ostrom’s principles as the precedent for a tested design set. A co-author must be able to state each rival as a prediction that differs from the VSM’s, not as a competing vocabulary. Take the sub-themes in spec order, with one exception: read 4.7 (Ostrom and Cox) immediately after 4.1 and 4.2, because Cox et al. is the paper’s methodological precedent (§3.4) and the reader should hold it in mind while reading the rivals in 4.3–4.6.

The information-processing view of organization design

Introduction. Galbraith’s proposition is that the greater the uncertainty of a task, the more information must be processed among decision-makers during its execution, and that organisation structure exists to match information-processing capacity to that load. Uncertainty is defined as the gap between the information a task requires and the information the organisation already has. The design options fall into two groups: reduce the need for processing (slack resources, self-contained tasks) or increase capacity (vertical information systems, lateral relations from direct contact through liaison roles, task forces and teams to matrix structures). Each has a cost, and the organisation that chooses none of them defaults to the first, slack, without noticing. The view descends from Simon’s bounded rationality and March and Simon’s account of organisations as information-processing structures, and it consolidated the contingency tradition of Burns and Stalker, Lawrence and Lorsch, and Thompson into a single design variable. Its later development (Tushman and Nadler; Daft and Lengel on media richness; Burton and Obel’s computational fit models) kept the frame and added measurement.

Important authors. Jay R. Galbraith (1939–2014) was on the faculty of MIT Sloan and the Wharton School, later at the University of Southern California’s Center for Effective Organizations and IMD, and then an independent consultant; his Star Model is the practitioner form of the theory. Paul Lawrence and Jay Lorsch (Harvard Business School) and James D. Thompson supplied the contingency base. Michael Tushman (Harvard) and David Nadler restated the view as an integrating concept; Richard Burton and Børge Obel (Duke; Aarhus) formalised it; Phanish Puranam (INSEAD) is its current microstructural heir.

Importance for cybernetics and the VSM. Galbraith’s view is the closest mainstream competitor to the VSM’s explanation of structure: both say that structure is a response to the mismatch between variety in the task environment and the capacity to handle it, and both prescribe attenuation (reduce the need) or amplification (increase capacity). Galbraith arrived there through Simon rather than Ashby and does not cite Beer; the VSM literature does not cite Galbraith. The difference is that Galbraith’s options are enumerated, costed and comparable, and his framework has forty years of contingency-theory measurement behind it. That makes it a rival operationalisation, not merely a parallel.

Importance for the article. §3.3 cites Galbraith (1973, 1974) in the first paragraph of the neighbouring literatures, the paragraph Applying new lenses (C5 §VIII.2) called a convenience sample; v02 keeps the citation and adds the survey around it. The substantive use is in Demonstration II (§7.2): rival (2) for claim IIa is “an information-processing-capacity model in Galbraith’s sense” that predicts regulatory adequacy at least as well as the VSM’s channel-capacity operationalisation at lower cost. §11.4 lists Galbraith’s account among the fields that may supply measures and counterhypotheses rather than wholesale rivalry. A reviewer from organisation design will ask why the paper treats the VSM’s channel principles as the primary claim and Galbraith as the rival, rather than the reverse, given the relative evidence bases; the co-author should be able to say that the receipt is symmetric — if Galbraith’s model predicts as well at lower cost, the VSM’s channel principles are retained as heuristics (§7.4).

Sociotechnical systems

Introduction. Sociotechnical systems (STS) theory holds that the technical and social organisation of work are interdependent and must be designed jointly; optimising either alone degrades the whole. The founding study is Trist and Bamforth’s account of British coal mines after mechanisation: the longwall method broke small, self-selecting, multi-skilled work groups into single-task shifts, and the result was absenteeism, inter-shift conflict and defensive individualism traceable to the mismatch between the new technology and the social system it displaced. Later Tavistock work found composite longwall arrangements in which semi-autonomous groups regained control of the whole task, with better outcomes. From this came the principles of the tradition: the work group as the unit of design, minimal critical specification, control of variances at source by those who experience them, redundancy of functions rather than of parts, and the open-system view of the enterprise. Emery and Trist’s “causal texture” paper added a typology of environments by turbulence. The Norwegian industrial democracy projects and Scandinavian work redesign applied the approach at scale; Mumford’s later assessment records where it succeeded and where it lost ground to technology-led design.

Important authors. Eric Trist (1909–1993), social psychologist and a founder of the Tavistock Institute of Human Relations, later at UCLA, Wharton and York University; Ken Bamforth, a former miner and Tavistock research fellow. Fred Emery, Trist’s principal collaborator, moved the work to Australia. Albert Cherns (Loughborough) codified the design principles; Enid Mumford (Manchester) applied them to information systems; William Pasmore (Columbia) wrote the American textbook.

Importance for cybernetics and the VSM. STS grew up alongside cybernetics, shared the open-systems vocabulary, and its “control of variances at source” is requisite variety applied to the shop floor. The VSM literature cites Trist and Bamforth as a neighbour and rarely goes further. Two points bear on the model. First, STS is an empirical tradition: it began with field consequences of a design and built its principles from them, which is the order the VSM did not follow. Second, semi-autonomous groups are System One autonomy with a documented record, including the record of where the design was abandoned — a body of evidence on the VSM’s own prescription that the VSM has not used.

Importance for the article. §3.3 gives STS one sentence: “Sociotechnical research examined autonomous groups (Trist and Bamforth, 1951),” in the paragraph v02 concedes was a convenience sample; §11.4 lists sociotechnical design among the fields that may supply measures and boundary disciplines. There is no demonstration built on it. The value for a co-author is defensive and comparative. A reviewer from the Tavistock tradition will note that STS produced controlled comparisons of work designs (composite versus conventional longwall) fifty years before the paper’s stage-six “comparative interventions” (§10.5), and will ask why the VSM’s autonomy claim is not first tested against that archive. The honest answer is that it should be, and that a retrospective receipt on the composite-longwall comparison would be a stage-one entry at near-zero cost. The co-author should also be ready to say what STS’s principle of redundancy of functions adds to the degeneracy discussion (§3.3, §9): it is the same idea, stated for work design in the 1960s.

Organizational ecology, inertia and the survivorship rival

Introduction. Organisational ecology explains the diversity of organisational forms by selection at the population level rather than by adaptation of individual organisations. Hannan and Freeman’s 1977 paper set out the programme: founding and mortality rates, niche width, the specialist–generalist distinction and competition among populations, with formal models borrowed from bioecology. The 1984 paper supplies the mechanism. Selection favours organisations that are reliable (low variance in performance) and accountable (able to justify their actions), both achieved by reproducible routines and structures; reproducibility produces structural inertia; inertia rises with age and size; and attempts at core change reset the liability of newness and raise mortality hazard in the short run. Organisations therefore rarely adapt successfully and populations change mainly by replacement. The consequence for any theory of design is survivorship bias: the organisations available for study are the survivors, and whatever they have in common may reflect selection rather than the merit of the feature. Later work (Carroll and Hannan) formalised density dependence and organisational demography; Denrell showed how undersampling of failure produces false management lessons; Levinthal showed that a random walk reproduces the age-dependence of mortality without any adaptation.

Important authors. Michael T. Hannan (Stanford Graduate School of Business, emeritus) and John Freeman (1944–2008; Berkeley Haas) founded the field in sociology and later published the book-length statement together; Glenn Carroll (Stanford) is Hannan’s long-term collaborator on organisational demography. Arthur Stinchcombe’s 1965 essay supplied the liability of newness that the theory builds on. Jerker Denrell (Warwick) and Daniel Levinthal (Wharton) provided the two sharpest survivorship results, from opposite directions: undersampling of failure and mortality without adaptation.

Importance for cybernetics and the VSM. Organisational ecology is not a competing design theory; it denies that design is what survival tracks. That makes it the rival every VSM viability claim must name: an organisation that persists and displays the five functions may persist because it was selected, and the five functions may be what selection left rather than what caused persistence. The VSM has no population-level concept, no mortality hazard, and no account of inertia — the properties that make an organisation reliable are, on Hannan and Freeman’s account, the ones that make it unable to do what System Four and Five are supposed to do. Structural inertia is on the list of pathologies with no VSM name.

Importance for the article. §3.3 opens the rival mechanisms with “survival is selection, not design” and states that no v01 receipt named the survivorship alternative; §11.3 now requires every local receipt to record “always, the survivorship rival that the organization’s persistence reflects selection rather than design”; §3.3’s pathology list adds structural inertia; and the stage-four archival study in §10.5 (a VSM-diagnosed cohort against matched controls on company mortality, using Daepp et al.’s constant-hazard result) is the only design in the paper that could adjudicate between design and selection. The import is Critique Claude (C1 §10: “the live rival the paper never names”) and Applying new lenses (C5 §VI). Both papers are Tier C, marked †. A reviewer will press on whether the cohort study escapes selection into treatment — VSM-diagnosed organisations are those that sought diagnosis, often in trouble — and on whether §11.3’s “always” is enforceable. The co-author should state the matched-control design and concede that the selection problem needs an instrument the paper has not specified.

Learning, exploration/exploitation and competency traps

Introduction. March’s 1991 paper frames organisational learning as an allocation between exploration (search, variation, experimentation, whose returns are distant and uncertain) and exploitation (refinement, efficiency, whose returns are near and reliable). Two models carry the argument. In the first, individuals learn from an organisational code and the code learns from the individuals who outperform it; fast learners converge on the code quickly and the code stops improving, whereas slow learners and turnover preserve the diversity the code needs. In the second, competition for primacy rewards variance rather than mean performance. The central result is that adaptive processes refine exploitation faster than exploration, so they are effective in the short run and self-destructive in the long run. The competency trap (Levitt and March) is the mechanism: experience with an inferior procedure raises its returns enough that a superior one is never tried. Later work located the balance in structure: Lazer and Friedman showed that efficient communication networks converge faster and reach worse solutions; Fang, Lee and Schilling showed that semi-isolated subgroups with occasional cross-links outperform both fully connected and fully isolated designs; the ambidexterity literature asks whether one organisation can do both.

Important authors. James G. March (1928–2018), Stanford, earlier at Carnegie Tech with Simon and Cyert, founded the behavioural theory of the firm and the study of organisational learning under ambiguity. Barbara Levitt co-authored the competency-trap paper; Daniel Levinthal (Wharton) co-authored “The myopia of learning” and carried the theme into landscape models; David Lazer (Northeastern) and Melissa Schilling (NYU Stern) produced the network results; Charles O’Reilly (Stanford) and Michael Tushman (Harvard) lead the ambidexterity line.

Importance for cybernetics and the VSM. The exploration–exploitation trade-off is a rival to the Three–Four homeostat: where the VSM treats the balance between present operations and future adaptation as something a designed homeostat can hold, March shows that ordinary adaptive learning drifts toward exploitation on its own, and that the drift is accelerated by fast convergence — which is what effective coordination produces. The VSM’s tacit “more coordination is better, up to the autonomy constraint” reading of System Two therefore has a documented adverse consequence: premature convergence and competency traps, neither of which has a VSM name.

Importance for the article. §3.3’s pathology list opens with “competency traps (March, 1991†),” and the candidate receipt in §10.5 — “More coordination (System Two) is better, up to the autonomy constraint” against “optimum connectivity is interior and horizon-dependent” — pairs March’s mechanism with Derex and Boyd’s experiment as the discriminator. The import is Applying new lenses (C5 §IV and §VI: “premature convergence on a local optimum, which the VSM’s coordination machinery would accelerate”). March is Tier C, marked †. A reviewer will press on whether the VSM in fact prescribes maximal coordination (Beer’s autonomy principle says otherwise) and whether the two readings predict different data. The answer is that the receipt targets the tacit teaching, not Beer’s text, and that the discriminator — cumulative-innovation experiments with VSM-specified coordination regimes — predicts an interior optimum for System Two connectivity that shifts with time horizon. Fang, Lee and Schilling is the closest organisational evidence and belongs beside Derex and Boyd.

Organizational silence and voice

Introduction. The silence-and-voice literature asks why employees withhold information, concerns and suggestions from those above them, and what follows. Morrison and Milliken (2000) defined organisational silence as a collective phenomenon: a climate in which employees believe that speaking up is futile or dangerous, produced by managerial beliefs (employees are self-interested, management knows best, dissent is unhealthy) and sustained by structures and practices that discourage upward communication. Milliken, Morrison and Hewlin (2003) interviewed forty employees and found most had withheld a concern, that the dominant reason was fear of being labelled negatively and damaging relationships, and that silence is rational from the sender’s position. Morrison’s 2014 review consolidates antecedents (perceived futility, fear, efficacy, leader openness), distinguishes voice from silence as constructs, and concedes that systematic evidence on the organisational effects of silence is sparse. Adjacent lines are Hirschman’s exit–voice–loyalty, the MUM effect (reluctance to transmit bad news), Edmondson’s psychological safety, and Detert and Burris on whether leaders’ apparent openness changes voice. The literature’s strength is its mechanism; its weakness is the outcome link.

Important authors. Elizabeth Wolfe Morrison and Frances J. Milliken, both professors of management at New York University’s Stern School of Business, founded the organisational-silence line; Patricia Hewlin (McGill) co-authored the interview study. Amy Edmondson (Harvard Business School) leads the psychological-safety programme; James Detert (Virginia Darden) and Ethan Burris (Texas) the leadership-and-voice line; Linn Van Dyne (Michigan State) the construct work. Albert Hirschman’s 1970 book is the older root.

Importance for cybernetics and the VSM. The VSM’s theory of why bad news fails to reach the top is architectural: the channel is missing, blocked or under-capacity, and the remedy is a designed algedonic route. The silence literature locates the failure at the sender, before any channel: the channel may exist and be open and people still do not use it because of the anticipated managerial response. These are different mechanisms with different remedies, and a VSM diagnosis of “channel failure” cannot distinguish them without a discriminator. Beer’s design specifies the channel and says nothing about the sender’s exposure — who is authorised to fire the signal and what happens to them afterwards.

Importance for the article. §10.5’s candidate table sets “escalation rate is a function of channel design” against the voice-climate rival, with incident and near-miss reporting data across organisations with and without designated algedonic routes as the discriminator, and the v02 note requires the caveat that the rival’s outcome evidence is thin. §13.1 uses the same literature as one count on which the governing metaphor fails its own audit: algedonics “is contradicted by fifty years of organizational evidence locating the failure at the sender rather than in the channel.” §11.3 adds the sender’s exposure to the practitioner’s standing statement. The import is VSM about Neuroscience (C3 §8), which supplied the sources and warned that “presenting it as settled would reproduce precisely the error the paper exists to correct.” A reviewer will press on “fifty years” (the silence literature dates from 2000; the MUM effect from 1970) and on the outcome link. Soften §13.1 to what Morrison (2014) will bear, and state the receipt as a rival with a strong mechanism and a declared evidential weakness.

Accidents, reliability and the failure literatures

Introduction. Four bodies of work describe how organisations fail. Normal accident theory (Perrow) classifies systems by interactive complexity (unplanned, non-linear interactions) and coupling (how tightly one step follows another); where both are high, accidents are consequences of system properties rather than pathologies, and the organisational requirements are contradictory, because complexity demands decentralised discretion and coupling demands centralised control. Perrow explicitly judges contingency management — decentralised in normal times, centralised under challenge — infeasible. The disaster-incubation model (Turner) shows, from British public inquiries, that disasters are preceded by a period in which discrepant events accumulate unnoticed or are misread because they conflict with prevailing beliefs; the disaster is the collapse of those beliefs. Vaughan’s Challenger study shows normalisation of deviance: evidence of O-ring damage was incrementally reclassified as acceptable by engineers following NASA’s own rules, aided by structural secrecy. High-reliability organising (Weick and Sutcliffe; the Berkeley group of La Porte, Roberts and Rochlin) claims that some organisations — carriers, nuclear plants, air traffic control — sustain both autonomy and tight coordination through mindful practices, which normal accident theory says cannot be done. Reason’s Swiss-cheese model and Leveson’s control-theoretic STAMP are the engineering counterparts.

Important authors. Charles Perrow (1925–2019), Yale, organisational sociologist. Barry Turner (1937–1995), Exeter; Nick Pidgeon (Cardiff) co-authored the second edition. Diane Vaughan (Columbia). Karl Weick (Michigan) and Kathleen Sutcliffe (Johns Hopkins). Todd La Porte, Karlene Roberts and Gene Rochlin (Berkeley) formed the original HRO group; Scott Sagan (Stanford) tested HRO against NAT on nuclear-weapons safety. James Reason (Manchester) and Nancy Leveson (MIT) represent the safety-science side.

Importance for cybernetics and the VSM. Perrow is the best-defined empirical disagreement with the VSM in the whole critique series. Beer claims a structure that reconciles autonomy and cohesion: System One autonomy in normal operation, algedonic escalation and System Three intervention under challenge. That is contingency management, and Perrow considered it and rejected it on accident evidence. The VSM literature does not engage him. Turner and Vaughan supply pathologies the VSM cannot name — incubation (failures accumulating below any monitored boundary) and normalisation of deviance (the threshold an algedonic signal fires against is itself corrupted) — and HRO is the one tradition that claims Beer’s design is feasible, without having been asked. Leveson’s STAMP is a control-theoretic accident model the VSM community could have produced and did not.

Importance for the article. §3.3 cites Perrow (1984) for the infeasibility of the arrangement the VSM prescribes where complexity and coupling are both high, and adds normalisation of deviance (Vaughan†) and incubation (Turner†) to the pathology list; §10.5’s candidate table sets “autonomy in normal operation with escalation under challenge is a viable design” against normal accident theory and high-reliability organising, discriminated by an adversarial receipt on cases in the complex-and-tightly-coupled quadrant. Perrow is Tier A; Turner, Vaughan and Weick are Tier C, and Weick and Sutcliffe is absent from v02’s references although HRO is named in the table. The imports are Applying new lenses (C5 §VI, “Perrow is the decisive challenge”) and VSM vs Complexity Science (C6 §2), which adds a third party: Siggelkow and Levinthal’s alternation result answers Perrow by sequence rather than balance and treats fixity as the pathology. A reviewer will press on whether Perrow’s “infeasible” is a finding or a judgement (a judgement, argued from cases), whether any VSM application sits in that quadrant, and how the three-way disagreement is staged; Sagan (1993) is the existing HRO–NAT adjudication to name.

Institutional analysis and the commons

Introduction. Ostrom’s Governing the Commons argued, against the standard prediction that common-pool resources are either privatised, nationalised or destroyed, that resource users devise durable self-governing institutions. From cases of irrigation systems, fisheries, forests and groundwater basins that endured or collapsed she extracted eight design principles: clearly defined boundaries; congruence between rules and local conditions; collective-choice arrangements; monitoring by accountable monitors; graduated sanctions; conflict-resolution mechanisms; minimal recognition of rights to organise; and, for larger systems, nested enterprises. The principles are context-sensitive and were offered as regularities, not laws. The later programme formalised the Institutional Analysis and Development framework, added the social-ecological systems framework, and studied polycentric governance. Cox, Arnold and Villamayor-Tomás (2010) coded ninety-one empirical studies against the principles, found them broadly supported, and reformulated three (boundaries, congruence, monitoring) by splitting each into two because the evidence bore on distinct components. Baggio and colleagues later showed that the principles operate configurally — success depends on combinations, not counts. Anderies, Janssen and Ostrom’s robustness framework connects the tradition to the resilience vocabulary of §2.1.

Important authors. Elinor Ostrom (1933–2012), Indiana University, co-founder of the Workshop in Political Theory and Policy Analysis, Nobel Memorial Prize in Economic Sciences 2009. Michael Cox (Dartmouth) and Sergio Villamayor-Tomás (ICTA-UAB, Barcelona) led the 2010 review. John Anderies and Marco Janssen (Arizona State) developed the robustness and modelling side; Arun Agrawal (Michigan) wrote the critical review of the enabling-conditions approach; Amy Poteete co-authored the methods book.

Importance for cybernetics and the VSM. Ostrom did not use Beer’s vocabulary, though nested enterprises and polycentricity are often compared with recursion. The importance is methodological. The eight principles are the closest thing in social science to the VSM: a context-sensitive set of design regularities derived from cases and offered for use. Unlike the VSM’s, they were then coded against the literature, scored study by study, and revised where the evidence split them. That is the programme the paper proposes, executed, for a comparable object — and its one warning, that a supportive coded literature contained no negative study, is the reason the register’s negative-results provision must be enforced.

Importance for the article. §3.3 cites Ostrom (1990) in the neighbouring-literatures paragraph and places the eight principles in the rival-set table; §3.4 uses Cox et al. (2010) as the first of two bounding precedents: “the programme this paper proposes, executed,” plus the cautionary finding that not one of ninety-one studies was moderately or strongly negative, read as “a signal about the underlying literature rather than about the principles.” §10.4 (negative and null results, enforced) rests on that reading. The import is Critique Claude (C1 §10: “your precedent, not your neighbour… Steal the coding protocol”). Cox et al. is Tier A. A reviewer will press on whether the coding protocol transfers to a model whose claims are not stated as principles (§4.1’s claim-type taxonomy is what makes them codable), and whether the VSM applications literature, coded the same way, would show the same absence of negatives — itself a cheap receipt. Baggio et al.’s configural result also bears on whether “identifiable but not separable” is a testable property of a principle set.

What you should be able to say after this theme

  • Galbraith’s information-processing view is an independent rediscovery of the variety logic with enumerated, costed design options, and it stands as the named rival operationalisation in Demonstration II.
  • Sociotechnical research tested autonomous work groups in the field decades before the VSM, and its composite-versus-conventional comparisons are a retrospective receipt on System One autonomy at near-zero cost.
  • Every VSM viability claim has a survivorship rival — persistence reflects selection, not design — and §11.3 requires every local receipt to name it; the cohort study in §10.5 is the only design that could adjudicate it, and it has an unsolved selection-into-treatment problem.
  • Ordinary adaptive learning drifts toward exploitation, fast convergence accelerates the drift, and the tacit “more System Two is better” teaching therefore predicts competency traps; the receipt targets the teaching, and the discriminator is an interior, horizon-dependent optimum for coordination.
  • The silence literature locates failed escalation at the sender, before any channel; it is a rival with a strong mechanism and a thin outcome link, and §13.1’s “fifty years” should be stated at the strength Morrison (2014) will bear.
  • Perrow’s judgement that contingency management is infeasible where complexity and coupling are both high is a direct contradiction of the VSM’s core design, the VSM literature has not engaged it, and the HRO–NAT–alternation disagreement is to be staged as a three-way adversarial receipt with Sagan (1993) as the template.
  • Incubation, normalisation of deviance, structural inertia and competency traps are documented pathologies with no VSM name, and their absence from a two-hundred-item model-derived taxonomy is evidence about how the taxonomy was built.
  • Cox et al. (2010) is the programme executed for Ostrom’s principles; its finding of no negative studies is why the register must enforce, not encourage, negative results.
